Position Summary
The PATH (Projects for Assistance in Transition from Homelessness) Team Lead oversees day-to-day operations of the PATH team, ensuring the delivery of high-quality outreach, engagement, case management, and service coordination to individuals experiencing homelessness who have serious mental illness, co-occurring substance use disorders, or other disabling conditions. This role ensures compliance with SAMHSA PATH Program requirements, HUD HMIS Data Standards (FY 2024), and state PATH reporting guidelines, including accurate data entry and timely submission of reports such as the PATH Annual Report and PDX uploads.
Key Responsibilities
- Supervise PATH-funded outreach and supportive services staff, providing regular coaching, training, and performance feedback
- Ensure staff understand and correctly apply definitions such as contact, engagement, enrollment, services, and referrals as outlined in PATH guidance
- Coordinate daily outreach routes, engagement strategies, and service delivery to meet client needs and contract performance targets
- Oversee HMIS data entry to ensure accuracy, timeliness, and completeness in line with FY 2024 HMIS Data Standards
- Monitor adherence to PATH data collection workflows, including recording Date of Engagement, PATH Status, PATH-funded Services, PATH-funded Referrals, and SOAR connections.
- Review and approve monthly and quarterly reports for internal monitoring and state / federal submissions
- Support team in providing trauma-informed, person-centered outreach and case management to individuals experiencing homelessness.
- Guide staff in linking clients to mental health services, substance use treatment, housing assistance, and mainstream benefits.
- Promote integration of SOAR-trained staff for SSI / SSDI application assistance when applicable.
- Maintain strong partnerships with community agencies, shelters, behavioral health providers, and Coordinated Entry systems.
- Represent the PATH program in local Continuum of Care (CoC) and interagency meetings.
Qualifications
Education
Bachelor’s degree in Social Work, Psychology, Public Health, or related human services field requiredMaster’s degree preferred (MSW, MA in Counseling, MPH, or related)Experience
Minimum 5 years’ experience in homeless outreach, behavioral health, or supportive housing servicesAt least 1 year of supervisory or team lead experienceExperience with HMIS or other client data management systems; PATH HMIS experience preferredKnowledge of HUD and SAMHSA PATH program requirements strongly preferredSkills & Competencies
Strong leadership, supervision, and conflict-resolution skillsExcellent organizational and time-management abilitiesCompetence in HMIS data entry, reporting, and compliance monitoringAbility to work with individuals with serious mental illness, substance use disorders, and co-occurring conditions in non-traditional settingsCommitment to trauma-informed care, harm reduction, and Housing First principlesLicenses & Certifications
