Job Title : Registered Nurse - Long-Term Care (LTC)

Position Overview :

Navitas Healthcare, LLC is seeking Registered Nurse - Long-Term Care for an exciting Travel or Local job in Rifle, CO .

Responsibilities :

• Provide direct nursing care to residents, including assessments, medication administration, and treatment plans.
• Monitor and document vital signs, medical conditions, and changes in resident health.
• Supervise and guide nursing assistants and support staff to ensure quality care.
• Participate in care plan development, updating, and implementation for each resident.
• Maintain accurate and timely documentation in compliance with facility policies and regulatory standards.
• Educate residents and families on care plans, health management, and procedures.
• Assist with resident transfers, mobility, and activities of daily living (ADLs).
• Support infection control practices, safety standards, and quality improvement initiatives.
