Intake Coordinator / CNA

Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator to join our team. This role is essential in ensuring patients have a smooth transition into home health and hospice care.

Job Requirements :

• CNA Certificate.
• Completing insurance forms and answering any patient inquiries.
• Scheduling home health / hospice visits efficiently.
• Handling daily patient referral & intake operations.
• Verifying insurance coverage and assisting the billing department with processing claims.
• Ensuring the seamless transition of patients to home care.
• Completing insurance forms and answering any patient inquiries.
• Answering, screening, and directing phone calls to staff; taking messages and scheduling appointments.
• Serving visitors by greeting, welcoming, and directing them appropriately.
• Receiving mail, documents, packages, and courier deliveries, and delivering or distributing items.
• Keeping a record of medical supply inventory.
• Selecting and packaging DME supplies.
• Assisting colleagues with administrative tasks.
