About the Role
The Center for Human Development (CHD) is hiring an Assertive Community Treatment (ACT) Coordinator / Therapist to support individuals with severe and persistent mental illness (SPMI) in maintaining stability and independence in the community.
You’ll provide direct therapy and supportive services, coordinate care with other providers, and oversee implementation of the ACT model of care.
