Bilingual Transitions of Care Associate

A company is looking for a Bilingual (Spanish / English) Transitions of Care Associate.

Key Responsibilities

Call patients post-discharge to check on their well-being and resource needs

Coordinate services and care with affiliated vendors for members

Assist in scheduling post-discharge visits with primary care and specialist providers

Required Qualifications

Medical Assistant Certification

2-3 years of experience in healthcare, managed care, home health, or community health settings

Fully bilingual in English and Spanish
