• Evaluate and assess patient to determine their appropriateness for care\n
• Utilize the available tools and resources to develop patient specific care plans with the appropriate level and frequency of care\n
• Communicate with the care team, physician, payors, patients and families to address care needs and fulfill the patients care plan\n
• Deliver high quality skilled care and supervise the care of other care team members\n
• Thoroughly document care delivery daily in our EMR system\n
• Contribute to a culture caring through individual accountability and teamwork\n\nSkills for Success : \n
• Compassionate in care delivery, focused on results\n
• Solution-driven, self-motivated, and responds with urgency\n
• Love learning, motivating, and inspiring patients to reach their goals\n
• Enthusiastic about working to the highest level of the RN license\n
• Familiar and comfortable with technology. HCHB experience is a plus\n\nExperience to Deliver on Our Mission : \n
• Current RN License, valid state driver's license, and reliable transportation\n
