This physician will provide compassionate, patient-centered care to individuals with complex chronic conditions such as CKD / ESRD, COPD, and CHF. - Deliver palliative care services to patients in the community through home visits and tele-health
 - Manage symptoms affecting quality of life coordinate referrals, educate care teams and track clinical and financial outcomes in collaboration with Medical Directors and interdisciplinary teams

 - Experience with all populations and payer mixes including high-need medicare, medicaid and commercial
 - Comfort and skilled in care discussions, advance care planning, symptom management, specifically complex medication panels, acute and behavioral health care and appreciation of social determinants
 - Licensure or ability to become licensed in required state(s)
