A leading national healthcare organization is seeking a full-time, board-certified Palliative Care Specialist to join its growing team. This physician will provide compassionate, patient-centered care to individuals with complex chronic conditions such as CKD / ESRD, COPD, and CHF. • Deliver palliative care services to patients in the community through home visits and tele-health
• Manage symptoms affecting quality of life coordinate referrals, educate care teams and track clinical and financial outcomes in collaboration with Medical Directors and interdisciplinary teams

• Experience with all populations and payer mixes including high-need medicare, medicaid and commercial
• Comfort and skilled in care discussions, advance care planning, symptom management, specifically complex medication panels, acute and behavioral health care and appreciation of social determinants
• Licensure or ability to become licensed in required state(s)
