Local Contract Nurse RN - Care Manager - $40-50 per hour

IDR Healthcare is seeking a local contract nurse RN Care Manager for a local contract nursing job in Fond du Lac, Wisconsin. Job Description & Requirements Specialty : Care Manager Discipline : RN Start Date : ASAP Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Local Contract Position : Registered Nurse (RN) – Case Manager (Home Health / Hospice) Location : Fond du Lac, WI and surrounding counties Start Date : ASAP Contract Length : 13 weeks (with potential to extend) Guaranteed Hours : 40 hours / week Shift : Monday–Friday, 8 : 00 AM – 4 : 30 PM Weekend / On-Call : None Serve as a Case Manager supporting home health and hospice patients across Fond du Lac, Dodge, and Columbia counties. This is a field-based role, requiring travel between patient homes and occasional facility visits. The RN will perform comprehensive assessments, coordinate care plans, collaborate with interdisciplinary teams, and manage end-of-life symptom support for patients and families. Responsibilities & Unit Overview Perform 5 patient visits per day in a home or facility setting Conduct comprehensive assessments and care planning Manage care transitions and coordinate with MDs, SWs, chaplains, CNAs, and facility staff Educate patients and families on symptom management and end-of-life signs Manage wound care, pain control, and medication education Maintain accurate and timely documentation in the EMR Engage with families and patients to ensure compliance with care plans Act as patient advocate, ensuring privacy and continuity of care ✅ Requirements Active RN license in the state of Wisconsin AHA BLS certification Valid driver’s license, reliable vehicle, and current car insurance Strong wound care experience required Must be comfortable working independently in a field-based role Experience with frail elderly and end-of-life care strongly preferred Minimum 2 years of Home Health / Hospice experience preferred Must be willing to complete skills checklist and provide proof of recent compliance training Coverage area includes Fond du Lac, Dodge, and Columbia counties Mileage reimbursed at $0.53 / mile EMR : Home Care Home Base (preferred); MatrixCare or other EMR acceptable Candidates without HCHB or hospice experience will be trained at a reduced bill rate until competencies are completed Training hours required will vary based on prior experience; typically ~16 hours for HCHB Required skills checklist and compliance documents must be uploaded before start Scrub color : not specified This is an onsite field role – not remote About IDR Healthcare IDR Healthcare is an awarding winning staffing firm that believes it is a privilege and awesome responsibility to serve those who selflessly care for others.
