- ???? What You’ll Do in This Role :
The Concierge provides participant-care services in the day center, participants’ homes, contracted placements, and vendor offices. Operating one-on-one or in small groups, this role supports individualized supervision, psychosocial engagement, homemaking, meal-planning, transportation coordination, clinical-appointment accompaniment, and documentation. The Concierge also backs the home-care department, day-center recreation and aide teams including transportation and kitchen, and administrative staff, while remaining adaptable to evolving operational needs.
Primary Responsibilities :
Bus Support :
Assist participants with transportation needs, including ride-alongs and safe loading / unloading.Provide door-to-door service excellence and support safe, comfortable travel.Center-Based Participant Services :
Support participants with ADLs, ambulation, and one-on-one supervision.Assist with transportation within the center (to clinic or therapy appointments).Supervise meals and engage participants in daily activities.Help integrate new participants and promote a positive, inclusive environment.Monitor safety and sanitation throughout the facility.Clinical and Medical Support :
Provide on-site support for clinical appointments (e.g., vision, dental, podiatry), including reminders, participant prep, and documentation.Accompany participants to community-based medical appointments as needed.Program Facilitation and Documentation
Assist with life enrichment programs to promote participant health and well-being.Maintain accurate documentation of all services and interactions.Team & Operational Support
Provide backup coverage for various roles to ensure continuity of care and service.Respond to urgent staffing needs with flexibility and a team-first attitude.Participate in staff trainings, meetings, and safety updates.Promote a positive, participant-first and team-oriented environment aligned with the PACE mission and values.Adhere to all regulatory and organizational standards for quality and safety.✅ What You’ll Need :
High school diploma or GED, required
Reliable transportation and a valid Michigan Driver’s License, required.
Possess current CPR certification by AHA or willing and able to obtain as offered by LifeCircles.
At least one year of experience working with frail elderly or long-term care populations.
Ability to lift 35 pounds without assistance
