About The Position

Must-Have : Proficient in HomeCare HomeBase and Oasis

Description :

Provides coordinates and directs the provision of home nursing care according to physician orders based on agency policies and procedures.

Responsible for the competent application of the nursing process.

Supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Conducts home visits and attends periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average patient load is 5-8 patients per day.

Each type of visit and activity has a different point value contributing to weekly productivity expectations.

Contractor may be assigned between 5-10 patients per day.

Types of cases include Start of Care, Wound Care, and Follow up Visits.

First-time travelers are not accepted; recent specialty experience is required.

Weekend and on-call requirements vary and should be discussed during the interview process.

Territory covered includes home visits within a 75-mile radius.

Mileage is reimbursed at the IRS standard rate.

Daily productivity points expectation is 30 points, equating to about 40 hours per week.

All documentation must be completed within 24 hours of the appointment with the patient.

Delayed or incomplete documentation impacting billing and follow-up care will result in a cancelation.

All travelers must have used HomeCare HomeBase and Oasis within the last 6 months or complete a retraining course during compliance.

There are no guaranteed hours with this contract, and clinicians must accept every patient on their schedule regardless of location.
