Description\n\nRN Coordinator- Providence Home Services- Oregon\n\nSchedule : Full Time / Days\n\nRN Coordinator role consists of the planning and coordination of new referrals. The position will also provide phone triage of existing patients. The majority of the responsibilities will be carried out onsite in office. May provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living site. This is accomplished through collaboration and coordination with various providers, the patient and / or caregiver, the physician and the payor.
