We deliver 24 / 7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. What You'll Do

As a Pediatric Home Care Specialist (Paramedic Certified) with Imagine Pediatrics, you will in collaboration with an interdisciplinary team, facilitate in home delivery of episodic and longitudinal care. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including APPs, registered nurses, navigators, pharmacists, psychiatrists, social workers, and therapists. You will :

• Travel to members' homes, or wherever in the community our members are, in response to requests for acute and episodic medical, behavioral, and social needs.
• Work closely as a member of an integrated care team to provide high-quality, compassionate, and skilled care to our pediatric members and their families.
• Assist, in collaboration with other members of the integrated care team, with urgent needs for our members. Provide therapeutic intervention under the direct guidance of a physician.
• Assist in the engagement and onboarding of new members by helping conduct and facilitate clinical intake visits.
• Engage with members recently discharged from inpatient admission or recently seen in the Emergency Department to prevent readmission, and identify and solve root causes of readmission.

The role offers a base hourly range of $24 / hr - $30 / hr in addition to annual bonus incentive, competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
