Registered Nurse - Case Manager Position : RN Case Manager Location : Clinton, MD Shift : Night 3x12 (19 : 00-07 : 00), Every Other Weekend Start Date : 11 / 17 / 2025 Requirement : Minimum 2 years acute case management experience, prior travel preferred Certifications : BLS, Active MD RN License Skills & Experience : - Coordinate patient care from admission through discharge in an acute care setting - Perform utilization review using InterQual and Milliman guidelines - Manage discharge planning and care transitions - Collaborate with interdisciplinary teams to achieve safe and efficient outcomes - Maintain accurate documentation within the EMR - Provide education and support to patients and families
