Pre-licensed Social Worker- Palliative Care (PT- 0.45 FTE, Day Shift)

Position Summary :

Develops and maintains a comprehensive program of services for patients and their families and aid in the adjustment to living with a temporary or chronic physical and behavioral issues. This includes advocacy, community education, counseling / therapy, research, program development, evaluation, and any other assigned responsibility relating to patients and social work. Focuses on prevention and promotion strategies for the social and emotional development and wellness of patients and their families. Provides screening, brief intervention and referral expertise and diagnosis for patients and their families. Position will serve as a member of a multidisciplinary healthcare team to improve the screening and management of social, emotional and behavioral concerns and interrelated medical conditions for patients. This position is for licensure candidates who will be reporting to a supervising therapist for clinical case oversight in compliance with the Montana Board of Behavioral Health requirements.

Minimum Qualifications :

Required

Master degree in Social Work or another comparable program

Licensed clinical candidate in the state of Montana (SWLC, PCLC, MFLC)

Preferred : One (1) year of experience in medical social work, Palliative Care, or Hospice

Essential Job Functions :

Provides quality assessment, intervention and support services to meet the psychosocial needs of patients and documents in appropriate EHR.

Provides appropriate screening, diagnosis and brief intervention / treatment and referral for children with behavioral health needs.

Serves as the behavioral health specialist within the multidisciplinary healthcare team to improve the coordination, screening and management of social, emotional and behavioral concerns and interrelated conditions.

Collaborates with Bozeman Health team members, State of Montana and Gallatin / Park county partners to provide behavioral health consultation and identify workforce development opportunities.

Schedule Requirements

This role requires regular and sustained attendance.

The position may necessitate working beyond a standard 40-hour workweek, including weekends and after-hours shifts.

On-call work may be required to respond promptly to organizational, patient, or employee needs.
