Role : Nurse Practitioner (Accepting New Grads)

Job Duration : Full-time

Location : Eastchester, Bronx NY (Onsite) and other multiple location if open to relocate

Facility : SNF

Compensation : $135,000 – $160,000 per year (commensurate with experience)

Overview :

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team and provide high-quality care in a skilled nursing facility setting. Key Responsibilities :

Serve as the primary care provider for both long- and short-term residents.

Conduct comprehensive health assessments, physical examinations, and review of medical histories.

Collaborate with attending physicians, nurses, and facility staff to deliver coordinated, evidence-based care.

Build trusted relationships with residents and their families; educate and develop individualized care plans.

Ensure compliance with all federal, state, and local healthcare regulations and standards.

Maintain professional development through continuing education and participation in professional organizations.

Support a clean, safe, and respectful care environment aligned with organizational values.

Ensure accurate, timely, and thorough documentation of all patient interactions and care provided.

Qualifications :

Must be a Board-Certified Nurse Practitioner (NP).

Strong clinical skills and attention to detail.

Excellent communication and teamwork abilities.

Demonstrated commitment to patient-centered care.

Experience in a hospital or long-term care setting preferred (training will be provided).

Competencies in health promotion, infection control, medication administration, and pain management.
