Job Overview Physician (SNF, LTC, ALC) :

Compensation : $350,000 $400,000 / year

Location : Torrington, CT

Schedule : Monday to Friday, 9 : 00 AM to 3 : 00 PM

Join our client as a Physician (SNF, LTC, ALC) in Torrington, CT, providing primary and transitional care to adult and geriatric patients in skilled nursing facilities, long-term care, and assisted living communities. This flexible weekday role is ideal for Internal or Family Medicine physicians with experience in geriatrics, hospitalist care, or post-acute settings. You ll manage chronic conditions, collaborate with interdisciplinary teams, and help deliver high-quality, continuity-based care across multiple sites.

Responsibilities as the Physician :

• Patient Care Management : Provide high-quality primary and chronic care to adult and geriatric patients in SNF, LTC, and ALC settings.
• Clinical Oversight : Monitor and adjust treatment plans, perform in-person assessments, and ensure continuity of care across multiple facilities.
• Interdisciplinary Coordination : Collaborate with nursing staff, specialists, and family members to ensure cohesive and patient-centered care.
• Documentation & Compliance : Maintain timely and accurate EHR documentation in accordance with state and federal healthcare guidelines.
• Facility Rounds : Travel to multiple partner facilities across Connecticut to provide hands-on care, clinical oversight, and patient follow-ups.
