Job description

Wilson Care is seeking an experienced and compassionate Registered Nurse (RN) Case Manager Lead  to join our growing team in our brand-new office ! This is an exciting opportunity to lead and inspire a team while making a meaningful impact on the lives of our clients.

Responsibilities :

  • Oversee and lead a team of case managers to deliver exceptional care.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans.
  • Monitor compliance with care standards, policies, and regulations.
  • Act as a primary resource and mentor for team members.
  • Manage client cases, ensuring their needs are met with the highest quality of care.
  • Maintain accurate records and documentation.

    • Qualifications :

  • Valid Registered Nurse (RN) license in Hawaii.
  • Minimum of 3 years of case management experience.
  • Leadership or supervisory experience preferred.
  • Strong organizational, communication, and interpersonal skills.
  • Passion for delivering exceptional care and leading a team.
