The nurse will provide high-quality care to patients in their homes, while also assisting with oversight of clinical operations, ensuring compliance, and supporting staff nurses in the field.\nJob Details : \n

- Deliver direct patient care in the home setting, including assessments, care planning, medication administration, and patient / family education\n
- Support the DON with administrative duties such as chart reviews, obtaining physician orders, and ensuring accurate and timely documentation\n
- Provide clinical oversight to field staff nurses, offering guidance, mentorship, and support as needed\n
- Cover patient visits when necessary to maintain continuity of care\n
- Collaborate with physicians, patients, families, and interdisciplinary team members to ensure the highest quality of care\n
- Assist with training and onboarding of new nursing staff\n
- Participate in quality improvement initiatives and contribute to the overall success of the agency\nRequirements : \n
- Must have an active RN license in the state of New Hampshire\n
- 1 to 2 years prior experience\n
- Active CPR certification\n
- Valid driver's license and reliable transportation\n
