Description : Position Overview :

Med-Care Providers is seeking a compassionate and experienced Nurse Practitioner (APRN-CNP) to provide in-home primary care services to homebound Medicare and Medicaid patients throughout Las Vegas and surrounding areas. This role is primarily focused on house visits , with the opportunity to also handle urgent care visits via telehealth as needed .

This is a flexible position ideal for providers seeking full-time or 2–3 fixed days per week with a consistent schedule.

Responsibilities :

• Conduct comprehensive home visits for elderly and homebound patients
• Evaluate and manage chronic and acute conditions
• Perform medication reconciliation, lab reviews, and coordinate with interdisciplinary teams
• Respond to urgent care needs via telehealth , as scheduled or on-demand
• Complete accurate and timely documentation to support compliance and billing
• Collaborate with care coordinators and specialists when needed
