Direct Hire
Location : Sacramento, CA
Travel : 10%–25% travel to participants’ homes and to cover at other facilities as needed. (Mileage is reimbursed and drive time is included in hours worked)
Compensation : $44.65 - $58.08 per hour (INCLUDES a $5,000 Sign on Bonus)
Start Date : ASAP
Schedule : Monday–Friday, 8 : 00 AM – 4 : 30 PM

Requirements
- Bachelor’s Degree in Physical Therapy
- Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association, and the APTA or other equivalent organization approved by CMS
- Active Physical Therapist license issued by the state of practice
- Minimum of one year of experience working with the frail or elderly

The Physical Therapist provides clinical evaluation and treatment plans for program participants aged 55 and older, on an acute, restorative, or maintenance level as needed. They are responsible for integrating physical therapy treatment plans into the Plan of Care (POC) as directed by physician orders, ensuring all participant care is delivered in compliance with applicable policies, procedures, and state regulations.
Responsibilities include :
Screen and evaluate participants for rehabilitation needs and follow APTA code of ethics in daily practice.Collaborate with nursing homes and assisted living staff regarding rehabilitation needs.Refer participants to Occupational Therapy (OT), Speech Therapy (ST), or medical clinics when appropriate, and update changes in the POC.Communicate with staff, family members, and caregivers regarding positioning, mobility, and safety needs, ensuring recommendations are feasible.Provide physical therapy treatment including modality treatment, therapeutic exercise, and functional mobility training, along with written instructions and illustrations as needed.Educate participants and caregivers on the use and maintenance of Durable Medical Equipment (DME), ensuring proper documentation of loans, returns, and repairs.Delegate therapy plans to Physical Therapist Assistants and Rehab Aides in compliance with state regulations.Make home visits as indicated in the POC and document activities, including caregiver responses.Actively participate in interdisciplinary team discussions to integrate services into overall care plans.Document all procedures in participant charts promptly and accurately.Provide training and guidance to staff on participant-specific needs and safe mobility practices.Order, maintain, and track DME in alignment with budget and departmental goals.
