Job description

At West Gardena Post Acute we are always looking for outstanding nurses to join our team to be able to carry out the functions and responsibilities that coincide with high quality patient care that comprises of, safety, customer service, quality outcomes and over all satisfaction.

We are currently hiring for the position of LVN Treatment Nurse and Charge Nurse here at West Gardena Post Acute. Experience is a must.

Position is for AM shift (7AM-3PM), for Saturdays & Sundays.

Essential Functions

  • Must be able to knowledgeably and competently deliver quality nursing care to patients
  • Must be able to identify, manage, and treat specific skin disorders and primary / secondary lesions
  • Must be able to work with nursing team to educate CNAs as to proper positioning, training, and skin care
  • Must be able to evaluate patients’ needs through ongoing assessment and revise treatment plan through coordination with physician and interdisciplinary care team (IDT)
  • Must exhibit excellent customer service and a positive attitude towards patients
  • Must demonstrate dependable, regular attendance
  • Must be able to concentrate and use reasoning skills and good judgment
  • Must be able to communicate and function productively on an interdisciplinary team

Further Responsibilities :

Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition

Assist with the development and implementation of individualized care plans based on residents' needs and goals

Coordinate care with other members of the healthcare team, including physicians, therapists, and other healthcare professionals

Ensure that all documentation and charting is accurate, complete, and up-to-date

Assist with activities of daily living (ADLs) as needed

Educate residents and families about their healthcare needs and treatments

Respond to emergencies and provide first aid as needed

Adhere to all policies, procedures, and regulations governing nursing care in the facility
