Location : Lane County, Oregon, Eugene / Springfield area Setting : Outpatient Clinics (could be assigned to any of six different clinics across the county), Outpatient only Schedule : M-F, 8a-5p, No Call, nights, weekends, holidays; Ongoing Start Date : ASAP End Date : Ongoing Requirements :

• Board Eligible (minimum) FM / IM
• Active, unrestricted OR license or obtain OR license; New OR license process 8-10 weeks once app is received - Cost : $245
• Clean DEA (no current suits, unresolved cases, large payouts)
