As a Home Health RN , you'll deliver high-quality skilled nursing care, complete comprehensive assessments, and ensure documentation accuracy in accordance with agency policies and state / federal regulations. This is a field-based role requiring clinical visits within the designated coverage area. What We Offer Competitive compensation $1,000 sign-on bonus Weekly pay with direct deposit 401(k) with 4% company match $500 referral bonus for qualified candidates Purpose-driven, patient-centered work environment Key Responsibilities Provide skilled nursing care and coordinate patient services per individualized plan of care Complete OASIS assessments accurately and in a timely manner Utilize Homecare Homebase (HCHB) for documentation and visit management Monitor and report changes in patient condition to the care team and physician Administer medications and treatments as prescribed Educate patients and caregivers on disease processes, medications, and home safety Participate in case conferences, quality reviews, and performance improvement initiatives Maintain compliance with all agency, state, and federal requirements Qualifications (Required) Active Pennsylvania RN license Current BLS certification (American Heart Association) Minimum 1 year of Home Health experience Proficiency with OASIS and Homecare Homebase (HCHB) Reliable transportation, valid driver's license, and proof of auto insurance Preferred 2 years of home health or post-acute nursing experience Strong clinical assessment and critical thinking skills Experience providing patient and family education Ready to take the next step in your RN career?
