Job description

Permanent Dietitian – PACE (Program of All-Inclusive Care for the Elderly)

Location : Sioux City, IA

Schedule : Monday–Friday, 8 : 00 AM – 4 : 30 PM

Employment Type : Full-Time, Benefits Eligible

FLSA Status : Non-Exempt

Hourly Rate : $28.00 – $40.00 per hour

Sign-On Bonus : Up to $10,000

Overview

A PACE organization dedicated to supporting older adults in maintaining independence and quality of life is seeking a Permanent Dietitian to join our interdisciplinary care team. The Dietitian is responsible for assessing nutritional needs, developing individualized care plans, implementing interventions, and evaluating outcomes to ensure optimal health and well-being for program participants.

Under the supervision of the PACE Program Manager, this role contributes to a collaborative approach to care that aligns with the PACE model's mission and values.

Key Responsibilities

  • Maintain a working knowledge of all applicable PACE Federal, State, and local regulations, as well as organizational policies, procedures, and ethical standards.
  • Obtain and evaluate participants’ nutritional and medical histories, laboratory data, test results, medications, and dietary regimens to determine nutritional needs.
  • Develop, recommend, and implement nutrition interventions to manage or improve nutrition-related medical conditions.
  • Complete comprehensive nutrition assessments per regulatory guidelines and as needed to support participants’ individualized care plans.
  • Educate participants, caregivers, staff, and contracted providers on diet, nutrition, food safety, and sanitation.
  • Accurately document assessments, plans, interventions, and progress in the electronic medical record (EMR).
  • Participate in daily Interdisciplinary Team (IDT) meetings and contribute to coordinated care planning.
  • Maintain professional competency through continuing education and adherence to current dietetic standards of practice.
  • Perform other duties as assigned.

Qualifications

Education & Experience :

  • Bachelor’s or Master’s degree in Dietetics, Nutrition, or related field required.
  • Completion of an accredited Dietetic Internship is required.
  • Experience with geriatric or chronically ill populations preferred.
  • Prior experience in a PACE program or long-term care setting is a plus.
  • Licensure / Certification :

  • Registered Dietitian (RD / RDN) credential required.
  • State licensure (or eligibility for licensure) required.
  • Current CPR certification preferred or ability to obtain within six months of hire.
  • Skills & Attributes :

  • Excellent communication, assessment, and documentation skills.
  • Strong ability to collaborate within an interdisciplinary team environment.
  • Compassionate, patient-centered approach to care.
  • Proficiency with EMR systems and Microsoft Office Suite.
