Description :
The Care Coordination Social Worker (CC SW) is responsible for assessing transitional care needs.
The CC SW coordinates care across the continuum.
Engagement with patients and families is essential to assure care needs are met.
The role serves as a key resource for patients and as a consultant to care team members regarding psychosocial and resource needs.
In conjunction with the patient and physician, the CC SW will assess, coordinate, and implement a timely, safe patient discharge plan.
This plan will transition to the next appropriate level of care.
The role integrates and coordinates the patient’s transitional care plan into individualized discharge plans based on needs and resources available.
The CC SW is responsible for providing psychosocial assessments for patients.
They ensure timely and appropriate planning to advance the discharge plan.
The CC SW assists in relaying information about community-based service offerings.
This includes guidance to patients and families to assist with multi-system factors affecting psychosocial dynamics.
They serve as a specialist on psychosocial and discharge needs, end-of-life care planning, and resource needs.
The CC SW will provide resource information necessary to aid patients and families in decision-making.
They will partner and offer feedback to the RN Case Manager concerning complex social determinants of health issues.
The CC SW initiates assessment of patients’ psychosocial risk factors based on preliminary screening.
They collaborate with patients, families, physicians, and other care team members to establish and support discharge plans.
The CC SW implements discharge planning and provides resource information effectively and efficiently.
They document barriers for timely disposition and understand eligibility processes for private and public resources.
The CC SW responds to referrals for patient assistance from the care team.
They participate in interdisciplinary rounds to confirm estimated discharge dates and make recommendations for transitions.
The CC SW advocates for patients and families, empowering them to make autonomous healthcare decisions.
They account for cultural and religious beliefs while providing services and continuity of care.
The CC SW completes all initial and ongoing professional competency assessments and supports departmental goals.
They may serve as a preceptor for social work students if appropriate.
Additional duties may be assigned as needed.
