Care Coordinator - Full-Time (In-Office)

AccordCare Home Health , an accredited home health agency, is seeking an experienced Care Coordinator to join our Maitland, FL office. This pivotal in-office role ensures our team and patients are supported through efficient scheduling and coordination , specifically for Private Duty Home Care services .
Key Responsibilities
Schedule and coordinate all field staff visits in accordance with the Plan of Care for private duty clients
Work closely with the Clinical Manager to manage patient schedules
Maintain accurate records in the computer system with schedule changes and visit authorizations
Communicate with Intake, referral sources, sales staff, and team members to facilitate patient care
Collaborate with the Billing Department as needed
Facilitate payroll and participate in on-call rotation
Address and resolve patient / family scheduling concerns
Build and maintain strong relationships with staff, sales teams, and referral sources to ensure efficient service coordination
Qualifications
Minimum 1 year of Private Duty Home Care scheduling experience required
Strong written and verbal communication skills
Excellent customer service skills
Ability to work effectively in a fast-paced, office-based environment
Exceptional organizational and time management skills
