Position Overview :

Sun River Health is seeking a full-time Nurse Practitioner (NP) to join our team at the St. Ann's Health Center in the Bronx. The NP will work under the direction of a physician to deliver comprehensive, patient-centered healthcare, with an emphasis on preventative care and patient education. The NP will be responsible for planning, evaluating, and providing healthcare services to patients, ensuring the highest standards of care in alignment with Sun River Health's mission.

Key Responsibilities :

• Patient Care : Perform patient assessments, including taking medical histories and conducting physical examinations.
• Prevention & Education : Emphasize preventative health measures, offering education and counseling on health maintenance and lifestyle changes.
• Clinical Care : Order and interpret diagnostic tests, prescribe treatments for minor illnesses, and provide follow-up care. Draw blood and perform laboratory tests as necessary.
• Collaboration : Work closely with physicians and other healthcare providers to ensure high-quality care, referring patients as needed and consulting with other team members.
• Documentation : Maintain accurate and up-to-date patient records, ensuring appropriate documentation is in compliance with organizational standards and regulatory requirements.
• Patient Monitoring : Follow up on abnormal results, including abnormal pap smears and reportable STD's, ensuring timely patient care.
• Case Conferences & Quality Improvement : Participate in case conferences, continuing education programs, provider meetings, and quality improvement initiatives.
• Infection Control : Adhere to OSHA and CDC guidelines for infection control and safety standards, ensuring a safe clinical environment for staff and patients.
• Age-Specific Care : Provide care appropriate to the patient's age-specific needs, from infants to geriatrics.
• Teamwork & Leadership : Collaborate with the multidisciplinary team to meet productivity and quality standards, while maintaining a high level of patient satisfaction.

Position Type : Full-Time

Salary Range : $154,876.80 - $162,385.80 (based on experience and licensure)

Minimum Education & Experience Requirements :

Bilingual (English / Spanish) preferred, with proficiency in both oral and written communication.
