Mains’l is seeking experienced Direst Support Professionals who are interested in growing into Support Coordinators . As the Support Coordinator, you will collaborate with the Program Manager and Direct Support Professionals to achieve goals of the people supported while providing person-centered care.  The Support Coordinator will be a role model of how direct support is provided and connecting while leading and mentoring the Direct Support Professionals.

Location :   Oakdale, St. Paul, MN

Shift : Dependent on Location

Wage : $ 18.50 / HR

Job Responsibilities

• Meal planning
• Grocery shopping
• Budgeting and managing household bills
• Scheduling and accompanying individuals to doctor’s appointments
• Provide Person Centered Supports tailored to the individual needs (including Activities of Daily Living, behavioral supports, coaching through different situations, participating in home and community activities)
• Managing and administering medications
• Collaborate and communicate effectively with the team on service plans
• Documenting care accurately
• Ensure a safe and structured environment
• Assisting in scheduling, coaching, and training of Direct Support Professionals
