Job Summary and Responsibilities
- Patient population includes Geriatrics, Dementia, and Palliative Care
- Collaborate closely with interdisciplinary teams and Advance Practice Providers to enhance efficiency and care coordination to ensure comprehensive patient care
- Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
- Serve as a Geriatrics Consultant, including screening and diagnosing Dementia
- Clinic position with possibility of home visits
- Full time position, Monday through Friday
- Provider will have 4 hours of administrative time per week
- Daily patient volume : anticipated 6-10 patients per day
- Current programming includes multi-disciplinary Dementia Screening Clinic, Alzheimer's & Dementia Care Program (UCLA dissemination site), Outpatient and Inpatient Palliative Care, and GUIDE model participant
- Opportunity to enhance and expand services in the Cunningham Center for Compassionate Aging
Job Requirements
BE / BC in Geriatric Medicine (IM or FP), Neurology, or Hospice & Palliative Medicine (IM or FP)Experience or interest in working with Dementia / Alzheimer's Disease populationStrong communication and interpersonal skillsValid state medical license as MD or DOValid DEA certificateStrong clinical skills and experience working with older adultsAbility to work effectively in a team-based healthcare environmentDemonstrate empathy, compassion, and respect for the unique needs of older adults and their care partnersWhere You'll Work

Pay Range
$120.20 - $134.62 / hour
