Overview:

We are looking for a compassionate and organized Onboarding Specialist to join our home health care team. In this role, you will welcome new patients, ensure they understand our services, and provide a warm and supportive introduction to their care journey. You'll be the first point of contact for new patients and families-helping them feel comfortable, informed, and confident as they begin home health services.

Basic Function:

• Conduct welcome calls to new patients and introduce them to our home health services.
• Verify patient information and confirm their understanding of the care process.
• Address initial questions or concerns from patients and family members.
• Provide an overview of what to expect during their home health care experience.
• Ensure all onboarding documentation is completed accurately and in a timely manner.
• Communicate relevant patient details to nursing and administrative teams.
• Follow up with patients as needed to enhance their onboarding experience.
• Maintain detailed and accurate records of patient interactions.
• Coordinate care efficiently with ancillary companies and departments.

Requirements:

• High school diploma or equivalent required; associate or bachelor's degree preferred.
• Previous experience in home health, healthcare administration, or patient services preferred.
• Excellent verbal and written communication skills.
• Compassionate, patient-focused, and professional demeanor.
• Strong organizational skills and attention to detail.
• Proficient in Microsoft Office and comfortable learning new software systems.
• Ability to work both independently and as part of a team in a fast-paced environment.
