As a Home Health Physical Therapist, you will provide personalized physical therapy care to patients in their homes, helping them restore function, relieve pain, and prevent disability. You will develop and implement individualized treatment plans, document evaluations and outcomes, and consult with physicians and interdisciplinary teams to coordinate care. Your role includes educating patients and families about rehabilitation, maintaining dignity and independence, and driving to patients’ homes throughout the region. You will also participate in discharge planning and communicate with other healthcare professionals to ensure optimal patient outcomes.

Required qualifications include a Bachelor’s or Master’s degree in Physical Therapy from an accredited program, a current state license to practice Physical Therapy, and experience in home health or geriatric care is preferred. Strong interpersonal and communication skills, proficiency with electronic medical record systems, and the ability to work independently and manage a flexible schedule are essential. Advanced certifications in physical therapy specialties and multilingual capabilities are a plus. CPR, BLS, and AHA certifications may be required.
