Position Summary
We are seeking a dedicated and reliable Licensed Vocational Nurse (LVN) to join our skilled nursing team. As an LVN in home health, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with the plan of care. Visits may include wound care, medication administration, chronic disease monitoring, and patient/family education.

- Perform skilled nursing visits to patients in their homes in accordance with the care plan
- Accurately document assessments, care provided, and patient response in EMR (Synergy experience preferred)
- Monitor and report changes in patient conditions to supervising RN and physician
- Provide wound care, catheter care, injections, and medication management
- Collaborate with interdisciplinary team to ensure coordinated, patient-centered care
- Educate patients and caregivers to promote independence and safety
- Comply with Medicare/Medicaid and state regulations
Qualifications
- Current LVN license in the State of Nevada (unrestricted)
- One (1) year of clinical experience (home health preferred)
- Valid driver’s license, auto insurance, and reliable transportation
- CPR Certification (BLS)
- Ability to work independently and manage time effectively
- Proficient in electronic documentation (EMR experience preferred)
