Your Mission in Action
The Medical Coordinator arranges and coordinates appropriate health services, including urgent, emergent, and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments, coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers. This role adheres to agency policies, licensing, and regulatory standards.
Responsibilities
Coordinate initial medical screenings with primary care provider within ORR timeframesCoordinate routine health screenings and assessments, administer prescribed medication and treatment, coordinate medical appointments, and manage nutritional needs to ensure timely and comprehensive care for each UCCoordinates the administration of immunizations and TB screenings per ORR or local Health Department requirementsSupport infection-control policies and procedures as well as conduct staff training on medication dispensing and Psychotropic medicationsFacilitate trainings for staff, such as dispensing prescribed, over-the-counter and psychotropic medication, bloodborne pathogens, and CPR / First AidWork closely with the Program Leadership and staff to ensure UC are receiving the highest quality of care and the program is operating efficiently and effectively, and is meeting the goals and objectives of the program and the needs of the UCOther duties and special projects as assignedQualifications
Minimum Qualifications
Current LPN / LVN license in Texas or higher health–related qualification (e.g. licensed resisted nurse (RN) with bachelor’s degree) OR Bachelor’s degree in relevant fieldOne (1) year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical careHigh School diploma or equivalentBilingual English and Spanish, (speak, read and write)Must be 21 years of age or older per licensing requirementsStrong interpersonal, customer service, and relationship management skillsAbility to build trust with UC and internal stakeholdersStrong documentation, time management, organizational, and conflict resolution skills, with proficiency in Microsoft Office and UC information systemsPreferred Qualifications
One (1) year of experience with pediatrics and / or adolescentsAssociate degree in a medical or science fieldTravel
