Travel Registered Nurse - Hospice

We're looking for aHospice RN – Regional Support to join our team. In this dynamic role, you'll provide direct care to patients and support to families, while also serving as a floating clinician—temporarily assigned to different hospice branches based on census needs or staffing gaps. If you're energized by new settings, adaptable to change, and passionate about purpose-driven care, this role could be a perfect fit.

As a Hospice RN – Regional Support, You Will

• Deliver high-quality, compassionate nursing care for hospice patients
• Manage and implement individualized plans of care
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
• Assess and address patients' physical, emotional, spiritual, and environmental needs
• Accurately document all care provided and ensure compliance with hospice protocols
• Educate families and caregivers on care instructions, medications, and safety
• Participate in the on-call rotation and provide after-hours care as needed
• Collaborate with physicians, referral sources, and the care team
• Assist with referrals, regulatory documentation, and payer compliance
• Travel to various Gentiva hospice locations as assigned to fill urgent needs

Schedule & Travel

This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include :
