Monogram Health is seeking a full-time physician, specialized in palliative care, to join the team due to continued nationwide growth.
This physician will be an in-market specialist caring for the seriously ill population in their community, including patients with CKD / ESRD, COPD, and CHF. The Palliative Care physician will be integrated into their local market with the pod Medical Director and primarily provide care in the home and through telemedicine visits.

Roles & Responsibilities
Collaborate with the market Medical Director and care team to identify patients for palliative care and build a patient panelDevelop and present metrics to executive leadership as neededResponsible for clinical and financial outcomes for the patient panel, driving KPIs through interventionsTreat patients both in-home and virtually to improve healthcare outcomesEvaluate and manage symptoms affecting quality of life, such as pain, depression, anxiety, etc.Refer members to appropriate specialists and conduct goals of care and family meetings, documenting wishes in state AD / POLST formsSupport patients with our interdisciplinary team, including Palliative Care ChaplainAssist with hospice transitions as appropriateDevelop relationships with local providers to improve market alignmentParticipate in monthly meetings regarding KPIs and expectationsEducate and support the team on palliative care, hospice, advance directives, and communicationPosition Requirements
Desire to practice in a forward-looking renal care environment focused on population healthExperience applying evidence-based clinical criteriaSkilled in goals of care discussions and advance care planningExperience with Medicare Advantage, managed Medicaid, and commercial populationsUnderstanding of integrating acute and behavioral health care and social determinants of healthExpertise in managing medication panels for complex patientsCurrent unrestricted medical license and free of sanctions from Medicaid or MedicareWillingness to become licensed in multiple states as neededDoctorate Degree in MedicineBoard Certified in Hospice and Palliative MedicineReliable transportationAbout Monogram Health
