POSITION SUMMARY / RESPONSIBILITIES

In collaboration with case managers, physicians, staff, community health resources and payers, is accountable for assisting Social Workers and Case Managers with coordination of care secondary to patient / family psychosocial / socioeconomic needs across the care continuum. Ensures timely care, compliance with regulatory requirements / standards and effective / efficient use of hospital resources. Completes established competencies for the position within designated introductory period.

EDUCATION / EXPERIENCE

High school diploma required. Associate's degree preferred. Six months experience in a healthcare environment required. Preferred experience in HMO, hospital, home health or any clinical setting involved in discharge planning.
