About The Position

Job Title : Care Manager, Utilization Review RN

Profession : Nursing

Shift : Monday to Friday

Hours per Shift : 8am - 5pm

Experience : Minimum 3 years of clinical nursing experience performing direct patient care

License : Valid, unrestricted Professional Nursing License to practice as a Registered Nurse in any state of the United States or its territories

Certifications : Basic Life Support (BLS) certification by the American Heart Association (AHA)

Must-Have :

Associates Degree in Nursing (ADN), must obtain Bachelor’s in Nursing within two years of hire

Demonstrated knowledge of case management, and utilization review processes

Experience working with culturally diverse low-income populations

Travel nursing experience as a contractor

Description :

The incumbent serves as an Outpatient / Inpatient Case Manager / Utilization Review Registered Nurse.

The individual is responsible for direct supervision and supportive contact for identified patient assignments.

This role focuses on empowering clients to achieve demonstrable outcomes and self-sufficiency.

Responsibilities include assessment, service planning, resource acquisition, and monitoring client progress.

The position also requires performing Utilization Review and Clinical Documentation Chart Reviews.

Collaboration with multidisciplinary teams and participation in case management services is essential.

The individual will work closely with social work case managers on discharge planning and care transitions.

The role involves linking clients with community social service providers and healthcare resources.

Experience with barriers to self-sufficiency such as illiteracy, welfare dependency, domestic violence, and substance abuse is important.

Responsibilities should be executed in alignment with the director of care coordination.
