Exciting Opportunity : Join Healthcare United as a Primary Care Physician Healthcare United Pittsburgh, PA Healthcare United is actively seeking a Board-Certified Primary Care Physician (Internal Medicine or Family Medicine) to join our dynamic outpatient team in Pittsburgh, Pennsylvania . This role offers the opportunity to provide whole-person, team-based care in a collaborative and patient-centered environment. Position Highlights As a Primary Care Physician , you will : Deliver comprehensive primary care services , with a focus on managing chronic conditions and preventing complications. Work as part of an interdisciplinary care team including social workers, care managers, behavioral health specialists, nurses, pharmacists, and nutritionists. Treat acute exacerbations of chronic conditions onsite when appropriate, ensuring continuity of care. Offer same-day / next-day urgent appointments for patients recently discharged from hospitals, ERs, or skilled nursing facilities to prevent readmissions. Develop strong transitional care plans to track patients throughout their healthcare journey, including hospital discharges and post-acute care settings. Utilize evidence-based medicine and adhere to the Patient-Centered Medical Home (PCMH) model of care. Optimize technology and Electronic Medical Records (EMR) to improve efficiency and quality initiatives. Key Responsibilities Diagnose, treat, and manage patients with multiple chronic conditions . Educate patients on chronic disease management and healthy lifestyle choices . Monitor and achieve quality metric goals to improve patient outcomes. Attend multi-disciplinary team meetings to develop care plans and recommend treatment initiatives. Provide on-call coverage for the office and hospital in rotation with other providers. Requirements Licensed Physician in the state of Pennsylvania. Board Certification in Internal Medicine or Family Medicine. BLS Certification required. Experience in population health management is preferred. Strong background in managing psychosocially and medically complex patients is highly desirable. Familiarity with NCQA PCMH models preferred. Proficiency in Electronic Medical Records (EMR) and Microsoft Office Suite . Exceptional interpersonal and communication skills with a team-oriented approach .
