Internal Medicine Physician

Start Date : ASAP (up to 4 weeks credentialing) - 3 months

Desired Schedule : Monday - Friday from 8 : 00am-5 : 00pm (no call)

Scope of Work : Average of 5 patients per day; manage chronic disease states of geriatric population; must work with care plans within group of : NP, IM Doc, PT, OT, Dietician, and other specialties for each patient; manage dialysis treatment set up; coordinate all care for these patients daily

EMR : EMR is E-Clinical works (can train)

Requirements : Board Cert in IM / FM, CA license, DEA, BLS, Geriatric experience, PACE program or Team based practice experience, 2 references from DO / MD
