Job Description
American Traveler is seeking an experienced Physical Therapist for a home health position requiring at least one year of recent home health experience and a valid PT license.
Responsibilities
Position is based in home health settings serving patients throughout Pinellas CountyPatient population includes med / surg, cardiac, orthopedic (hips, knees, shoulders), post-surgical, neurology (including CVA, Parkinson's), respiratory, trauma, ADL needs, and moreCaseload of 6-8 skilled home care visits per day with a minimum of 6 points and manager discretion to increaseTreats Medicare, Medicare Advantage, and commercial insurance patientsDay shift schedule, 5x8 hour shifts from 8 : 00 to 17 : 00Requires 1-2 weekends per contractUse of BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system, and Cerner charting (preferred experience)Laptop, MiFi, and portable printer provided for home visitsSelf-scheduling of hours and visits based on the needs of the HomeCare officeDuties include comprehensive PT assessments, developing home exercise programs, providing patient and family education, monitoring for signs of abuse, and emphasizing safety and injury preventionResponsible for equipment use and recommendations including gait belts, walkers, lifts, collars, wheelchair assessment, supplemental oxygen, and prostheticsHome visits are self-scheduled based on client needs and office requirementsProfessional support includes access to technology needed for documentation and home care visitsSolid color scrubs are required100-mile radius rule applies; candidates must live at least 100 miles from the facility for eligibilityFacility is smoke-freeReturn travelers must wait at least one year after permanent employment to returnTraining modules must be completed pre-start and annually, recorded by the facilityNo RTO requests accepted in advance; time off must be worked out onsite with unit managerCandidates may be required to float within the hospital or to affiliate facilitiesRequirements
