Job Summary :

We are seeking a serious, compassionate, experienced, dedicated Registered Nurse (RN) to join our home health and hospice agency as the Director of Nurses. This position will require both field and office duties, with a focus on providing exceptional patient care, ensuring high standards of documentation, and leading a diverse team. The ideal candidate will be solution-driven, bilingual (preferred), and able to work effectively with different cultures, ages, and ethnic groups in a professional and inclusive manner. The role demands strong organizational skills, a high level of documentation, and a willingness to teach and mentor staff.

This position is for both hospice and home health. Experience in both is required to be considered for this position.

Key Responsibilities :

Leadership and Management :

  • Lead and manage the nursing team, providing guidance, support, and mentorship.
  • Develop and implement nursing policies and procedures to ensure high-quality patient care.
  • Foster a positive and inclusive work environment, promoting teamwork and collaboration.
  • Ensure the clinical staff is educated, and the development of such is ongoing.
  • Develop and implement processes to ensure an organized approach to daily tasks.
  • Willing to Learn new things and play a key role in the growth of the company.
  • Develop relationships in the clinical community and act as a source of education for non-clinicians for any marketing endeavors.
  • Ensure quality assurance and compliance of clinical notes and processes.
  • Participate with local and federal agencies to ensure successful surveys.
  • Encourage growth physically and intellectually.
  • Work closely with management in the hiring of talented staff to ensure the company vision and goals are met.
  • Patient Care :

  • Oversee and coordinate patient care plans, ensuring they meet the needs of each patient.
  • Conduct field visits to assess patient conditions and provide direct care as needed.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure quality patient care is delivered as a part of the everyday culture.
  • Documentation and Compliance :

  • Ensure accurate and timely documentation of patient care and medical records.
  • Maintain compliance with all regulatory and accreditation standards.
  • Conduct regular audits of patient charts and documentation to ensure quality and accuracy.
  • Adhere to all state and federal regulations and policies.
  • Quality Assurance :

  • Implement and monitor quality assurance processes to improve patient outcomes.
  • Identify areas for improvement and develop action plans to address them.
  • Participate in QA meetings and contribute to the development of policies and procedures.
  • Education and Training :

  • Provide education and training to nursing staff on best practices, policies, and procedures.
  • Mentor and support new staff members, ensuring a smooth onboarding process.
  • Promote continuous learning and professional development among the nursing team.
  • Cultural Competence :

  • We are a small agency specifically geared towards excellence in hospice for adults and children, this applicant should be open to spearheading the agency to be a community leader in this endeavor.

  • Qualifications :

    Education and Experience :

  • Experience in both home health and hospice is required.
  • Current and valid RN license in the state of employment.
  • Minimum of 2 years of nursing experience, with at least 1 year in a leadership role.
  • Working knowledge of Kinnser.
  • Experience working with or willingness to work with children to bring peace to hurting families..
  • Bilingual (preferred)- Stipend pay for Spanish speaker.
  • Able to audit and correct simple errors in clinical charts by communicating and teaching documentation to fellow clinicians within three days of the note's submission.
  • Thorough understanding of leading an IDT meeting while charting in Kinnser.
  • Able to rally participation in IDT meetings.
  • Effectively organize and lead a clinical staff in the field.
  • Effectively organize and lead patient coordination in the office for the benefit of authorizations and scheduling.
  • Able to audit agency processes and create QAPI reports quarterly.
  • Experience transmitting HIS and Oasis.
  • Effectively maintain 5-star level performance measures using state and CMS standards.
  • Skills and Abilities :

  • Strong leadership and management skills.
  • Excellent organizational and time-management abilities.
  • Knowledge of current Medicare guidelines and able to ensure compliance of clinicians through education.
  • Open to working with families with children and the ability to get these families clinical care either through personal care or human resource contacts.
  • Ability to work independently and as part of a team.
  • Kind and caring in nature but with the ability to lead by example.
  • Proficient using Kinnser.
  • Willingness to learn and apply new processes and practices to enhance the company’s objective to service the community through all ages and stages.
  • Ability to be the clinical face of A Hug Away Healthcare and lead us into a bright future caring for families with home health and hospice needs.
  • Additional Requirements :

  • Ability to handle sensitive and confidential information.
  • Ability to report to work on time with zero to few absences.
  • Flexible and patient with team members and patients.

  • Attendance at marketing events when possible.
