Schedule : Monday–Friday, 8 : 00 a.m.–5 : 00 p.m. (with occasional flexibility for events or coverage)
Role Summary
We are seeking a highly organized, proactive, and compassionate Patient Care Coordinator / Scheduler to join our growing team at Harmony at Home Senior Care. In this role, you will be the pivotal point of contact between clients, caregivers, and internal teams—ensuring seamless scheduling, excellent client experience, and accurate documentation. You will help ensure our seniors receive consistent, high-quality in-home care by coordinating shifts, managing schedules, and resolving staffing or client-service issues efficiently.
Key Responsibilities
Coordinate and schedule caregiver shifts for client assignments across the Dallas / Fort Worth Metroplex.
Serve as primary contact for clients and caregivers regarding schedule changes, call-offs, no-shows, and other staffing issues.
Monitor caregiver availability, ensure proper shift coverage, and manage substitution or rescheduling when needed.
Collaborate with service coordinators, field supervisors, and administrative teams to optimize staffing and client‐care alignment.
Maintain accurate and up-to-date records in our scheduling and CRM systems, communicate changes clearly, and ensure timely follow-up.
Respond to escalations or urgent client / caregiver issues, exercise judgement and escalate as needed.
Review scheduling metrics and trends (e.g., call-offs, coverage gaps) and recommend improvements.
Provide outstanding customer service to clients, families, and staff—demonstrating empathy, clear communication, and resolution-oriented mindset.
Qualifications
Minimum of 2 years of scheduling, coordination, or staffing experience (home care, healthcare, or staffing agency preferred).
Strong verbal and written English communication skills—comfortable handling clients, caregivers, families, and internal stakeholders.
Ability to manage dynamic schedules, multitask, and think on your feet—especially when last-minute changes arise.
Reliable, detail-oriented, and highly responsive—ready to handle changes and deliver solutions quickly.
Comfort with technology and scheduling / CRM software; willingness to learn new platforms.
Empathy for seniors and caregivers; ability to remain calm, professional, and solutions-focused in stressful situations.
High school diploma or equivalent required; associate or bachelor’s degree in related field a plus.
