Job descriptionProviding direct client services, including intake and assessment. Managing a caseload and documenting progress in APSNET and ETO. Monitoring risk and adapting service plans as needed. Referring clients to appropriate external services. Conducting home visits and telephonic communication weekly. Bachelor's degree in Social Work or related field. Minimum of 1 year of case management experience; 4 years preferred. Enrollment in MSW program is a plus. English proficiency; bilingual in English / Spanish is a plus. Strong communication, time management, and organizational skills. Proficiency in Microsoft Office and standard business technology. Physical ability to conduct fieldwork and lift / move up to 10 lbs.
A social services organization in New York is currently seeking two APS Case Managers to join their team. These positions involve 100% fieldwork across the Bronx and Manhattan boroughs. Ideal candidates will have experience in case management and a passion for community support.
About the Opportunity :
- Hybrid or Remote : 100% Fieldwork
- Shift : Day
- Schedule : Monday-Friday
- Hours : 9 : 00 AM-5 : 00 PM, 35-hour work week
- Setting : Temp-to-Perm (6 months temp, then conversion)
