Role Overview : Are you the kind of Social Worker who thrives with a roomful of spinning plates—and still takes the time to make everyone feel seen and heard? We’re looking for a Social Worker who is not only experienced and driven, but also outgoing, empathetic, and intuitive. In this role, you will lead our Long Term Care admissions and discharges, ensuring that our residents have the best possible experience at Birkwood Village. This position is ideal for a candidate who is passionate about high-quality, person-centered care in a for-profit setting. A candidate who can build strong relationships with residents, families, staff, and our wider community. Key Responsibilities :

• Review and manage referral admissions
• Coordinate and facilitate community discharges
• Communicate effectively and solve problems
• Verify insurance details
• Possess knowledge of Medicaid, Medicare, and other insurance policies
• Multi-task efficiently
• Maintain a positive attitude and a desire to serve

Qualifications :

Minimum of high school diploma, higher education preferred.

1-2 years of social work experience in a healthcare environment
