Role Description

This is a full-time on-site role for a Licensed Clinical Social Worker (LCSW), based in Eads, TN. The LCSW will be responsible for providing direct social work services, including case management and addressing mental and behavioral health needs. The role will involve client assessments, treatment planning, and collaboration with interdisciplinary teams to ensure comprehensive care. Additionally, the LCSW will engage in monitoring progress, crisis intervention, and supporting clients' overall well-being.

Qualifications

• Proficiency in Social Work and Medical Social Work practices
• Experience in Case Management for targeted care delivery
• In-depth knowledge and expertise in Mental Health and Behavioral Health services
• Excellent communication and interpersonal skills for client and team interactions
• Master’s degree in Social Work or related field and LCSW licensure
• Ability to work effectively in an on-site setting
• Familiarity with healthcare systems and resources is a plus
• Demonstrated commitment to ethical practices and cultural competence
