SUMMARY

Responsible and accountable for direct interaction and education of patients. Assist, educate and support patients regarding existing Substance Use Disorder Recovery Support in the communities and enhancing the ongoing therapeutic techniques offered in inpatient and outpatient Substance Use Disorder counseling and rehabilitation.

JOB DUTIES

Facilitate access to various community resources following completion of successful rehabilitative treatment to ensure clients' basic needs are met.

Offer referrals to Case Management services via patient's home county Single County Authority or other applicable agencies

Collaborate with clinical and support staff to address mutual client needs.

Act as liaison between the Stepping Stones Unit and Medical Director with other rural health clinics and programs and in providing support and encouragement to those referred to or participating in the OP Stepping Stones Medication Assisted Treatment / Medication for Opioid Use Disorder therapy.

Offers guidance on the recovery experience, including stage-appropriate recovery education.

Offers support and advocacy related to obstacles that are often encountered during the early stages of recovery.

Assist with case coordination needed for client needs.

Assist counselors in the development of individualized recovery plans for participants.

Assist participants in acquiring self-care skills such as budgeting, shopping, cooking, housekeeping, interview skills, etc., through educational groups and role plays and / or direct assistance.

Assist with follow-ups post-discharge to monitor participants' recovery stability.

Introduce and engage participants in the recovery community.

Provide ongoing assistance for participants in obtaining and / or maintaining services from multiple systems.

Arrange for transportation for participants as needed with assistance of primary counselor and approval from the Program Supervisor

Assist in coordination of outreach activities to engage new participants in the recovery process and skills for maintaining these contacts post-discharge

Perform follow-up activities appropriate for participants' aftercare plans until participants' recovery has stabilized.

Serve as a liaison to program participants for re-admission recovery concerns in accordance with program policies and re-admission / assessment processes.

Provide ongoing support to participants who have completed the program, as needed

Attends and participates in staff meetings, case conference and other job-related meetings, clinical functions and / or activities.

Solicits supervision about all issues which necessitate supervisory input / feedback.

Performs other tasks, duties, or functions relating to the achievement of overall goals & objectives of the Stepping Stones programs

Uses security wand with patients, visitors, and any others upon entrance to the unit as per protocol and as directed.

Escorts patients as directed to 12-step meetings within the building.

Performs other duties as assigned.

SPECIFIC JOB DEMANDS

Strength : Medium Work - Lifting, Carrying, Pushing, Pulling 20 - 50 Lbs. occasionally, 10 - 25 Lbs. frequently or up to 10 Lbs. constantly.

Stooping : Occasionally - Bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.

Reaching : Frequently - Extending hand(s) or arm(s) in any direction.

Handling : Frequently - Seizing, holding, grasping, turning, or otherwise working with hand or hands. Fingers are involved only to the extent that they are an extension of the hand, such as to turn a switch or shift automobile gears.

Fingering : Frequently - Picking, pinching, or otherwise working primarily with fingers rather than with the whole hand or arm as in handling.

Feeling : Frequently - Noting attributes of objects, such as size, shape, temperature, or texture, by touching with skin, particularly that of fingertips.

Talking : Frequently - Expressing or exchanging ideas by means of the spoken word to impart oral information to clients or to the public and to convey detailed spoken instructions to other workers accurately, loudly, or quickly.

Hearing : Frequently - Perceiving the nature of sounds by ear.

Near Acuity : Frequently - Clarity of vision at 20 inches or less.

Accommodation : Occasionally - Adjustment of lens of eye to bring an object into sharp focus. This factor is required when doing near point work at varying distances from the eye.

Color Vision : Frequently - Ability to identify and distinguish colors.

MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

High school diploma or G.E.D

Has and continues to maintain a minimum of 2 years of self-sustained recovery outside of a restrictive setting.

Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) certification through the Pennsylvania Certification Board.

Valid PA Driver's License and active automobile insurance.

Ability to obtain a satisfactory Pennsylvania State Police Criminal History check, a DHS (Department of Human Services) Child Abuse History Certification, and FBI Fingerprint-based Criminal Background Check.

No felony convictions in the last 5 years

Adheres to Meadville Medical Center and Stepping Stones' Ethical Code of Conduct.

Complies with the federal and state regulations on the confidentiality of alcohol and drug abuse patients as described in 45 CFR, Part 160 and 164, 42 CFR Part 2, Act 33, and Act 126 and PA Codes 255.5 and 257

WORKING CONDITIONS

Normal patient care environment with little exposure to excessive noise, dust, and temperature changes. A large part of work time is sitting, standing or walking. Stress is a large factor in the environment.

Subject to certain distasteful patient care activities and exposure to communicable diseases.

Scheduled hours of work may include weekends and / or holidays.

May be exposed to potential hazards such as chemical wastes, body wastes, fluids and other possible infectious material.

Subject to unpredictable actions of patients.
