Job description

Job Overview :

We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong background in Home Health. This role involves assessing patients' needs, developing individualized treatment plans, and facilitating their rehabilitation to improve daily living skills and overall quality of life.

Duties :

  • Conduct thorough evaluations of patients to determine their occupational therapy needs.
  • Develop and implement personalized treatment plans based on assessment results.
  • Provide therapy interventions to patients in their own homes.
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care.
  • Assist in discharge planning by providing recommendations for continued care and support.
  • Educate patients and their families about therapeutic techniques and home exercises.
  • Maintain accurate documentation of patient progress and treatment outcomes.
  • Stay updated with the latest practices in occupational therapy and participate in ongoing professional development.

Requirements :

  • A degree in Occupational Therapy from an accredited program.
  • Valid state licensure or eligibility for licensure as an Occupational Therapist.
  • Experience in geriatrics preferred.
  • Knowledge of anatomy and its application in therapy practices.
  • Familiarity with case management principles and discharge planning processes.
  • Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
  • Ability to work independently as well as part of a collaborative team environment.

    Job Type : Contract

    Pay : $60.00 - $75.00 per hour
