Job Overview :
We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have a strong background in Home Health. This role involves assessing patients' needs, developing individualized treatment plans, and facilitating their rehabilitation to improve daily living skills and overall quality of life.
Duties :
- Conduct thorough evaluations of patients to determine their occupational therapy needs.
- Develop and implement personalized treatment plans based on assessment results.
- Provide therapy interventions to patients in their own homes.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Assist in discharge planning by providing recommendations for continued care and support.
- Educate patients and their families about therapeutic techniques and home exercises.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Stay updated with the latest practices in occupational therapy and participate in ongoing professional development.
Requirements :
A degree in Occupational Therapy from an accredited program.Valid state licensure or eligibility for licensure as an Occupational Therapist.Experience in geriatrics preferred.Knowledge of anatomy and its application in therapy practices.Familiarity with case management principles and discharge planning processes.Excellent communication skills to effectively interact with patients, families, and healthcare professionals.Ability to work independently as well as part of a collaborative team environment.Join us in making a difference in the lives of our patients by helping them regain their independence through effective occupational therapy.
