We are seeking a Nurse Practitioner - Hospice - Pallative Care to become a part of our team! You will focus on providing high quality patient care as part of a healthcare team.

  • OH license (or willing to obtain)

Details :

Title :  Nurse Practitioner - Hospice - Pallative Care

Type : Full time Permanent

Facility Location :   1900 S Main St Findlay, OH 45840

Shift :   First shift, Monday–Friday, 8 : 00 AM to 5 : 00 PM

Compensation and Benefits :

  • Travel and reliable personal transportation required (no vehicle provided by the hospital)

    • Responsibilities :

  • Diagnose and treat acute, episodic or chronic illnesses
  • Prescribe all necessary medications and treatments
  • Perform comprehensive physical examinations of patients
  • Develop and implement patient management policies and procedures
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with collaborating physician or specialist regarding patient care

    • Qualifications :

  • Board Certified NP -  REQUIRED
  • OH license (or willing to obtain) -  REQUIRED
  • Experience as advanced provider  in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) -  REQUIRED
  • Clean malpractice and license -  REQUIRED
  • Date of agency's last communication with provider (put answer in "Additional Submission Details") -  REQUIRED
