Medical Records

Elizabeth Nursing and Rehab Center is seeking a detail-oriented and organized Medical Records Clerk to maintain and manage resident health records in compliance with federal, state, and facility regulations. The Medical Records Clerk plays a vital role in ensuring the accuracy, confidentiality, and timely availability of medical documentation for the nursing and administrative teams.

Key Responsibilities :

Organize, file, and maintain paper and electronic medical records for all residents

Review records for completeness and accuracy; follow up on missing or incomplete documentation

Ensure timely and secure release of information in accordance with HIPAA regulations

Assist with admissions and discharges by assembling and processing records

Coordinate with nursing and therapy staff to ensure timely charting and documentation updates

Audit charts for compliance with regulatory and facility standards

Prepare medical records for inspections, surveys, and audits

Track and log physician orders, lab results, and other clinical documentation

Respond to internal and external requests for health information, ensuring proper authorization

Support administrative staff with clerical duties as needed

Qualifications :

High school diploma or equivalent required

Experience in medical records or health information management (preferred)

Familiarity with HIPAA and healthcare privacy regulations

Strong attention to detail and organizational skills

Proficient in Microsoft Office and basic computer applications; EMR experience preferred

Ability to handle confidential information with discretion

Excellent communication and time management skills

Previous experience in a long-term care or healthcare setting is a plus

Schedule & Compensation :

Monday through Friday, daytime hours (flexibility may be required)

Competitive hourly wage based on experience
