JOB TITLE: Speech Language Pathologist – Home Health
DEPARTMENT: Home Health-Skilled
REPORTS TO: Clinical Manager
PAY: Pay per Visit
JOB SUMMARY:
Speech-language pathologists assess, diagnose, treat, and helps to manage/prevent communication and swallowing disorders in patients. This involves working on articulation, fliency, voice modilation, and cognitive-communication skills.
QUALIFICATIONS:
- Meets requirements for a Certificate of Clinical Competence in audiology from the American Speech-Language-hearing Association; or
- Meets the educational requirements for certification and is in the process of accumulating the supervised experienced required for certification
- NM State Speech Language Pathology license
- 1-year experience working in a health or hospice field preferred
- Valid state driver’s license
- Current automobile insurance

- Must be able to solve problems independently and confidently by applying analytical and logical thinking.
- Able to effectively communicate with individuals from diverse backgrounds and varying educational levels.
- Understands various medical conditions and terminology
- Demonstrated ability to document accurately using charting software
ESSENTIAL RESPONSIBILITIES:
- Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
- Interview the patient to determine type of treatment and therapy schedule.
- Perform speech therapy in patient's home.
- Monitors patient’s progress during treatment at appropriate intervals.
- Provide periodic progress records and report to assigned patient accounting to current policies and procedures.
- Supervising therapy assistants as appropriate.
- Evaluating outcomes. Planning Discharge.
- Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.
- Responsibilities may also include the training of students from university or community colleges assigned.
- Work with the agency's consultants as necessary and implement recommended changes as required.
- Participate in discharge planning, development and implementation of patient care plans, and assessments, etc., as necessary.
